On this page you will find the various forms used by Auckland Music Theatre members and helpers. Please select the form that you require and print out. Thank you.

Audition Form

Please fill in and bring with you when we have our next Show audition. Keep an eye out for the next announcement on our Facebook page (are you a member?),twitter follow us, e-mail us (do we have your e-mail address?), or keep coming back to the web site.

Expenses Claim form

Please print out and summit to show production Secretary or if not a show expense then to either the current Treasurer or a current Management Team member. Please attach receipts with all claims. As if no receipts are attached this could delay your reimbursement

Emergency Contact Form

We are required to have an Emergency Contact details in case anything happens to you while at Westpoint Theatre or any other venue that Auckland Music Theatre is using for the current production.
